• Credit hours for each course and term in which each course was taken.
• Copy of the first two pages of the most recently filed IRS Form 1040 used for the Financial Information Page (required). Please black out SSN.
• If a parent claimed you on their most recently filed taxes (2024), you must provide their financial information. If you filed your own taxes and are not claimed as a dependent, you will provide your financial information.
• One online recommendation form must be submitted on your behalf no later than three business days after the application deadline.
• Complete unless all required materials are submitted electronically.
